Directions for Submission

Self nominations by students who believe they meet the stated eligibility criteria must complete the webform with a brief statement of required qualifications and provide two letters of support from either faculty, staff, or community leader (not personal reference). Please note, if the student is being nominated, then only one letter of support is needed in addition to the nomination letter.
